Registry Participation
By creating an account, you confirm that the information you provide is accurate and that you consent to participate in registry workflows for joint replacement care, outcomes monitoring, and follow-up.
Account Responsibilities
You are responsible for keeping your login details secure and for informing your care team if your contact details, identity information, or clinical information need correction.
Clinical Use
The platform supports registry documentation, procedure tracking, PROMs collection, follow-up scheduling, and clinical review. It does not replace emergency care or direct medical advice from your doctor.
Consent and Withdrawal
Your consent is recorded with a timestamp and version. You may contact the registry administrator or your care team to ask questions about your participation or to request changes permitted by applicable law and registry policy.
